In the "Domain\Username" field, please enter your username as follows. "BERKELEY\username", where username is YOUR username.  e.g.

Username = jdoe
Domain\username = BERKELEY\jdoe

In the password field, please enter the password you use to log onto the Berkeley network. Once these two fields have been filled in, click the Log On button to continue.
